It is regrettable, but from time to time, the public may be victims of a notary’s malfeasance or malpractice. The Secretary of State’s office is committed to assuring the highest quality of professionalism and competence in our Notaries Public. If you believe that you have received inappropriate service from a notary, we want to hear about it.

Please download and complete this complaint form. Provide as much information as you can, particularly copies of the document that you believe was improperly notarized.
